Step 1: Download and complete the beneficiary form

  • The .PDF form can be found below to download
  • Please ensure the percentages add up to 100% or are blank to designate equal shares


Step 2: Sign the form

  • The form must be completed and signed by the primary member on the account
  • The form does not need to be notarized


Step 3: Return the signed form through Secure Email

  • Please send the signed copy through secure email by clicking on the +New Message icon above.
  • In the subject line, enter Beneficiary Form Request
  • Attach the form by selecting +Attach a file.
  • The information will be updated within 1 business day
